Scope the problem
A scoping call to agree the one problem worth four weeks, and what a defensible answer would look like.

Test the hypotheses
We set out what would have to be true for the problem to persist, then test each one against independent evidence from the work.

Rate the controls
Each critical control involved is rated across seven states, from designed to assured.

Report and present
A written report and an executive presentation: why the problem persists and what management should do about it.
The first state a control fails to reach is the finding, and it is almost never the first one.
