We work back from the event through absent or failed defences, individual and team actions, task and environmental conditions, and organisational factors.

Controls, not blame
We apply Human and Organisational Performance (HOP) principles: error is normal, context drives behaviour, and blame fixes nothing. Every finding is tied to a control, which one failed and why, so recommendations change the system, not just the behaviour of the last person involved.
